Surgical Indications and Results of Patients with CAPD-related Peritonitis



Abstract
As complications of continuous ambulatory peritoneal dialysis (CAPD) for chronic renal failure 17 out of 498 cases had developed compound abdominal pathologies mostly peritonitis and they were operated at Soon Chun Hyang University Hospital
between
July
1982 to August 1990.
Incidence of peritonitis is about 1.3 times per case per year, and the most of them were treated well either by appropriate antibiotics or by removal of the CAPD catheter, but the 17 cases did not responded well because of multiple intraabdominal
absceses (8 cases), hemorrhage (3 cases), or of intestinal obstruction (6 case).
Clinical symptomatologies were ambiguous, and operative decision was based on symptoms and signs of the intraabdominal pathologies but abdominal ultrasonogram examination offered the most useful informations.
Complications after the surgical intervention for the CAPD-related abdominal pathologies had also developed, 5 wound infection, 3 pulmonary complication, 3 intraabdominal absceses, and 2 intestinal obstruction.
3 mortalities occured, from sepsis, pulmonary edema and myocardial infarction respectively, thus mortality rates in 17.6% (3/17).
We feel early and prompt surgical exploration of abdomen when suspicious of CAPD-related compound intraabdominal pathologies to prevent fatal outcome and to low the mortality rate.
